Update May 6, 2021:
If you are a part of the NSFHT catchment area (Pays Plat to Jackfish), and over the age of 18, your consent form for the COVID-19 vaccine are needed as soon as possible. As we are not a part of the provincial booking system, this consent form is the only way for us to know that you are interested.
Consent forms can be completed and printed off of the North Shore Family Health Team website (http://www.northshorefamilyhealthteam.ca/covid-19-pandemic/) and dropped off to either J.E. Stokes Medical Clinic or The McCausland Hospital. Alternatively, you can email your completed consent to Cathy at email@example.com. Please be sure to send as an attachment and not embedded in the email.
If you are a student returning to the area from outside of the district, please indicate at the top of your form the date you arrived back so that we can calculate your 14-day isolation period and schedule your appointment appropriately.
If you have any questions, please call 823-3828 and leave a message and we will get back to you as soon as we can.
Who qualifies: Certain priority groups, including Indigenous 55+ and all people 50+
To register: The NSFHT is planning to contact people in certain priority groups once they have confirmed their next vaccine clinic dates. Those who would be eligible for vaccination during Phase 2 (e.g. Indigenous 55+, all people 50+) can complete a consent form http://www.northshorefamilyhealthteam.ca/covid-19-pandemic/ and return it to the doctors’ office or the hospital to prepare for when the clinics are scheduled.
